Introduction
If you are new to this practice, we ask that you attend about 10 mintues before your scheduled appointment so that you can fill out a form. We would like to know your age, address, contact details, and we have some questions about your health.
If you prefer, you can download the form, print it out and complete it in your own time. Just hand it to your receptionist when you come for your appointment.
Your personal information will never be divulged to anyone without your permission.
If there are any questions that you are unwilling or unable to answer, simply leave it and discuss with the dentist at your appointment.
Complex Treatment
Patients who require any complex treatment, such as sedation, difficult extractions, wisdom teeth, implants, long appointments etc, will be required to attend for a consultation beforehand. We will endeavour to provide as much information as possible, and to answer any questions that you may have.
Fees
We will give you an itemised estimate of your fees before your treatment appointment, and where this may be difficult, we will give the closest estimate possible.
